*All teammates must pay the Athletic Participation Fee.  This fee is listed in the eligibility book.  Before printing the above forms, be sure they are for the 2012-13 school year. 

*Each teammate will need the MSHSL eligibility form (to be completed once per year) and have a current physical on file (must be 3 years current throughout the sports season and on file with the Activities office).

*A Health Care Emergency Record - to be completed for EACH season.

*Academic Credit Requirements - these are located on the last page of the MSHSL Eligibility booklet. You must have the credit requirements for your grade in order to participate in an activity.

*Participation Fee Return Policy - participants of an activity have (10) practice days from the last date of an activity's start date, (or the participant's sign-up-date if later than start date), to resign from the activity and receive a refund of their participation fee.
