If you want to join an activity, follow the steps below and submit your completed forms to our Activities Office (forms are available for download from "Activities Documents").

1. Complete and Sign the Extracurricular Eligibility Bulletin
2. Submit Current Physical Exam Form (must be less than three years old at the end of this upcoming season).
3 Complete and Sign Health Emergency Care Record
4. Pay fee by check or cash. *If you qualify for a free or reduced price lunch, this fee will be adjusted or eliminated. Please contact the Activities Office for more information.
