2014 – 2015 SPORTS & FINE ARTS

REGISTRATION INFORMATION

Registration Dates:  Fall – July 28, 2014, Winter – October 1, 2014, Spring – February 1, 2015


To Register Online:

 

· On our website, under Activities, choose “Online Registration”.

· Then click on Register Online (blue box).

· Log into your A-H Connect parent account.  (Student log-ins do not work.)  If you do not have an A-H Connect parent login and password, contact the district at 763-506-4357.

· Next choose Activities/Transportation.

· Online payment options include VISA, MasterCard, Discover, and checking accounts.  There is no fee to register and pay your fee online.  All payments are processed at the time of registration.

· If you experience problems registering or paying online, contact help@feepay.com.

· REGISTRATION DEADLINE:  The Sunday prior to the start date of your sport.  (Football players must be registered by Thursday, July 31st to get uniform and equipment.)

· If you prefer to pay with cash (correct change only), you will need to complete the paper registration form and bring the form and your fee to the Activities Office.  See below for more details. 

· To register (for sports only), your sports physical cannot expire before the end of your sport’s season.  (See back of this flyer for the end date of your sport.)  A sports physical expires three years from the date of the examination.  

· If you need to update your sports physical, bring or fax it (763-506-7003) to the Activities Office.  Once submitted, you will be able to register online the next day.  If you need a sports physical form, they are available in the Activities Office or on our website under Activities (Forms to Download).

· Online registration is only available to Champlin Park students.  Middle school students, home school students, etc.) will need to complete the paper registration form and bring the form and activity fee to the Activities Office (see below).

 

To Register in Person:

 

· Complete the eligibility/registration form (pick up in the Activities Office or download from our website after July 28th).  Complete and return pages 7 and 8 of the Eligibility Bulletin.

· All payments are processed at the time of registration (checks are not held).

· All required paperwork and fees must be presented together at the time of registration.  Bring all forms and fees to the Activities Office.  If paying with cash, correct change only.  DO NOT give to your coach.  DO NOT MAIL.

FREE & REDUCED LUNCH STUDENTS

The participation fee will be waived if student qualifies for the district free lunch program.

Students on reduced lunch pay 50% of the full fee.  Students registering in the Activities Office

must present their letter of eligibility from Child Nutrition Services.

 

INDIVIDUAL & FAMILY MAXIMUMS PER SCHOOL YEAR

The individual max is $650.  When you reach the max, the online registration program will adjust the amount you owe.  The family max is $850.  The registration program cannot compute the family max. 

If you are reaching it, please register in person in the Activities Office.
