Home Education reporting forms to download The State Law requires that each home educator MUST register every year with the district. Registration documents are due October 1st. First time Home School Adminisrators must complete and submit the Full-Report and Immunization Records by October 1. Returning Home Schools must complete and submit a Letter of Intent to contintue (A template is available below) along with immunization information as required depending on the age of the students. The request for textbook aid form, 2012-2013 State Aids to Non-Public Students Form, is due September 15th.
