Home Education reporting forms to download

The State Law requires that each home educator MUST register every year with the district.

Registration documents are due October 1st. First time Home School Adminisrators must complete and submit the Full-Report and Immunization Records by October 1. Returning Home Schools must complete and submit a Letter of Intent to contintue (A template is available below) along with immunization information as required depending on the age of the students.

The request for textbook aid form, 2013-2014 State Aids to Non-Public Students Form, is due September 15th.

Description

The State Aids form is required if you wish to obtain reimbursement for educational materials purchased during the 2012-13 school year. This form is due September 15st annually. Late forms are not accepted. See the instructions for receiving textbook reimbursement or access to health services or guidance aid.

Note that on July 1, 2012, textbook aid expanded to include 'technology hardware'. Please see the document below to see how to claim reimbursement for those expenses.
