•  Friday, September 21, 2012, the online option for volunteers to complete the Criminal Record History Release form will be suspended.

•  Volunteers will still be required to complete release forms prior to starting their volunteer assignment; they will complete paper forms as in the past.  Paper forms (which will not require a person to enter their Social Security number) may be obtained from each school’s volunteer services coordinator or may be downloaded from the Volunteer page of the school’s website.

• There is no need for volunteers to complete a paper form if they already completed a form online.  On-line Criminal Record History Release forms already completed by volunteers will remain in the secure website of Trusted Employees and will still be accessed by Parent Involvement/Volunteer Services staff.
