On-line Criminal Record History Release Form

 

Thank you for considering volunteering with Hoover Elementary School. As part of our application process, we obtain information and permission to conduct background screens through Trusted Employees.  Please click on the link below (or cut and paste into your browser).

The Volunteer Application Form and Criminal Record History Release Form are available for you to print out at your convenience. If you print out the forms, please complete both forms and send them to school with your student attention "Volunteer Services Coordinator". Both forms are required each school year before you volunteer at Hoover.
