  Friday, September 21, 2012, the online option for volunteers to complete the Criminal Record History Release form will be suspended.

  Volunteers will still be required to complete release forms prior to starting their volunteer assignment; they will complete paper forms as in the past.  Paperforms(which will not require a person to enter their Social Security number) may be obtained from each school’s volunteer services coordinator or may be downloaded from the Volunteer page of the school’s website.

There is no need for volunteers to complete a paper form if they already completed a form online.  On-line Criminal Record History Release forms already completed by volunteers will remain in the secure website of Trusted Employees and will still be accessed by Parent Involvement/Volunteer Services staff.

To volunteer at CBPA, you must complete both the Volunteer Application and Criminal History Release form and return it to Cindy Dockendorf at CBPA.

Electronic Submission - The completed Volunteer Application form - Electronic Submission can be either returned with a click of the Submit button at the bottom of the second page OR, print and send it to the school VSC.
