• Friday, September 21, 2012, the online option for volunteers to complete the Criminal Record History Release form will be suspended.

• Volunteers will still be required to complete release forms prior to starting their volunteer assignment; they will complete paper forms as in the past. Paperforms(which will not require a person to enter their Social Security number) may be obtained from each school’s volunteer services coordinator or may be downloaded from the Volunteer page of the school’s website.

• There is no need for volunteers to complete a paper form if they already completed a form online. On-line Criminal Record History Release forms already completed by volunteers will remain in the secure website of Trusted Employees and will still be accessed by Parent Involvement/Volunteer Services staff.

Please download, complete and return BOTH the Volunteer Application form and the Criminal Record History Release form. Both forms must be on file before you start your volunteer assignment at Northdale Middle School.

New forms must be completed every year to comply with school district procedures.
